Sacramental Certificates
Requesting a record of a sacrament celebrated at our parish.
If you were baptized, received your First Communion, were confirmed, or were married here at Immaculate Conception, we are glad to provide a certificate from our parish records. This is often needed for godparent or sponsor eligibility, for marriage preparation, or for entry into the catechumenate. Complete the form below with as much detail as you can, and the parish office will locate the record and prepare your certificate.
Records are released in accordance with diocesan policy. Certificates are issued only to the person named on the record or to a parent or guardian, and may be sent directly to a parish for sacramental purposes. Please allow a few days for us to verify and prepare your request.
